Back in the day my old man extended the power lead for the TV like that. When we needed a repair the guy who came out wouldn't touch the set until he put the plug back on the original cable and moved the set nearer the socket.

After he left the bodge was reinstalled but this time covered with insulating tape.

I don't think he did it quite like it was done in the photo though.

I'm ashamed to admit it but I managed to do something very like that wiring up a foot switch into the lead of a standard lamp. As it was a trivial job with no difficulties I happily did it whilst tired. Obviously it instantly tripped the trip and when I opened it up to check it was very much a WTF moment. What I'd done didn't even make any sense never mind it being wrong.

For the avoidance of doubt I do understand electricity so it was not in any way lack of knowledge

Some years ago I was fitting an extension to a lawn mower - or some other garden electrical thingy - one of those which as one end with 2 sticky out bits
and the other end has 2 inward bit that the sticky out bits push into
(sorry about the overly technical terms!)

anyway - I wired it all up and tried the device out and all worked fine

I did whatever I needed to do with it and took it apart again
at which point I realised that I had wired up the sticky out bits on the house end and not the device end of the cable
which meant that once I had pulled them apart there were 2 live bits of metal sticking out

now - I assume that a fuse or breaker would trip it I put my hand (other body parts are available) across them but.....

anyway - I swapped them round and checked the instruction - which never mentioned which end goes where

a couple of years ago I had to do something similar with a different device and I noticed that the instruction showed which end goes where now

but it was still kinda small!!

anyway - just re-read all that and it does rather read like a sex manual
sorry about that
and after me being careful not to refer to bits as male and female!!!
