Given it was coupling up to the observation saloon the curvature of the end may have fooled the driver into thinking the tender end was further away. But, surely their should have been someone watching and clearly indicating to the driver.

Yes, there should of been either the shunter, or the fireman signalling the driver in. You are also supposed to stop 10 feet short, and then move in to couple.
