I'm a regular 9. and take a 44 in shimano. I've ordered a 43 and will advise on fit when I get them. Expect someone else might be able to provide a quicker response though.

Ordered some Fizik R3 shoes from Planet X yesterday and they arrived this morning! They are easily the most comfortable cycling shoes I've ever worn, like slippers but with a solid carbon sole. Love them.

I'm usually a 46 or 47 in shoes, but with cycling shoes I like a bit of room as when the weather turns cool it's an advantage. I would say the sizing on these is accurate.

Anyway, I went for 46.5 and they are perfect, magnificent shoes. They do have a high instep, though, but to me it felt like my feet were being cosseted, feel very comfortable indeed.

Just looked at my bike and they are all hex bolts so about half a dozen allen keys do the trick
