It SHOULD still happen occasionally no matter how long youve been riding, you only have to get the bike going the wrong way with your foot still clipped in and your snookered. Luckily it only happens when stopped or going very slowly
I don't know ... I still have near-fally-offy moments, such as misjudging barriers going into a park, or coming to a stop on gravel or mud a bit sooner than I expected and I always get my feet out in time. (fingers crossed)

Never had one in thirty years of use (I can safely say that now as I ride a recumbent trike).

I think my generation found the transition to clipless problem free. The reason being that everyone who raced and many who didn't used toe clips and straps with slotted shoe plates that fitted over the rear cage of the pedals. Procedure was to locate the slot with the shoe plate, lean down and pull the toe strap to lock your foot in the pedal. Before stopping, lean down again and flick the quick release buckle on the strap to free your foot. Clipless pedals were an absolute doddle to get used to after that and rather liberating, just a flick of the foot and you were out. I never knew anyone in the club I was in at the time having a problem either.

Different kettle of fish for anyone who has gone straight from having their feet unsecured, of course.

a moment is a combination of a physical quantity and a distance. For example, a moment of force is the product of a force and its distance from an axis, which causes rotation about that axis. In principle, any physical quantity can be combined with a distance to produce a moment; commonly used quantities include forces, masses, and electric charge distributions.
I fell off when my foot was stuck in the pedal is this the same thing?
