It might be that the cleats aren't angled correctly on your shoes and are trying to twist with every pedal turn. If you sit on a kitchen worktop and dangle your feet, they will point in a relaxed manner. Make a note of where each foot points and then adjust the cleat angles to replicate those when fixed to the pedals.
I have. The hard plastic of the rounded front of the cleat will wear away the oppositely shaped alloy of the pedal it engages with. Eventually the cleat will no longer engage. The alloy actually seems to wear faster than the cleat plastic!
