On every ride: Spare tubes, puncture kit, pump, spanner, multi-tool, First Aid kit, lock, camera, phone.

On longer rides I add: homemade energy drink, energy bars, compact lightweight waterproofs (rainlegs and a super compact jacket).

Yes Jaffa Cakes although not a full box. They seem to give me a welcome boost when needed.
 
Commute/NLTHWI:
2 tubes 700c only.
1 box Park Patches (to repair other inner tubes if necessary)
Multitool
2 x Bontrager tyre levers (only ever need one usually but a spare is handy in case of breakage)
4 surgical gloves (In case I'm passing a field and a cow needs help delivering a calf to avoid dirty hands - some people just don't know what 'clean my bike' means!)
CO2 +/- 3 cartridges - but I'll use a pump 'most' times, as I can get enough pressure.
Spanner for releasing the wheels

Basic Road Ride:

'Missing links' for 8 - 9 - 10 speed Campagnolo and Shimano

Lots of people road ride:
Chain tool
4+ tubes - repaired ones are good enough to get folk out of trouble if necessary.
Bits of tyre as 'boot'
More CO2, surgical gloves, batteries, spare gloves, buffs, gel, bits of snacky bars, full hip flask, whatever - in anticipation and depending on conditions etc...
Paracetemol, aspirin, Ibuprofen, zip ties, Pro plus, some gaffa tape or insulating tape.

Always I have a camera with me - how could I forget!

Damn it.
A Montaine Featherlite (tennis ball sized) jacket - occupies the spare bottle cage (rarely use them for bottles!)
Mobile phone is always there
Dosh + bank card.
St Christopher sewn into my camera case and a contact number in case I lose it. (Don't care about me but would miss my camera!)

I would like to know how phil_hg_uk gets all that into his wedge saddle bag. I have one, and I can't get anything like that much into it. The most I can manage is a tube, tyre levers, puncture repair outfit and a few tie wraps and it is full.

Gosh... I seem to be a bit of a slacker here...

1. Mobile phone, to call wife or cab if unable to proceed. Even typing that makes me feel like a pampered loafer....

2. Tube, levers, pump - usually for the correct valve type and tube size...

3. Water. I don't get on with that 'iso-doo-daa' stuff, but I drink a LOT of water.

Longer rides, as above plus:

1. Wallet

2. Gilet in case it gets windy or v wet.

3. Buttered fruit loaf squished into bag in pocket (Yummmmm)

4. 'Squashed Fly' biscuits for a rest from fruit loaf (Yummmmmm)

5. Banana - as part of my 5-a-day.

6. A couple of allen keys for major components and (on fixed-wheel) a 15 mm spanner for rear wheel.
 
I might add other things but my usual is,

in my wedge bag:
a multi tool (hexus with built in tyre levers);
a p'ture repair kit;
house keys;
and a rear led;

and in my jersey:
Spare Tube;
Pump;
Mobile;

Depending upon the length of the ride I'll carry food, water, a water proof jacket (Montane Featherlite), A4 printed maps and more lights and sometimes a small scrunch bag to carry unexpected loads home.
