Have you ever had perfectly-adjusted derailleurs?

Yes, on my Dawes. It has the advantage of ye olde friction shifters, allied with a modern chainset with pins and ramps. Everything is perfick.

Unless of course the tension on the right hand lever goes loose and it decides to make an unscheduled upshift just as I reach a hill.

Ive had a horrendous issue with my sram red recently ots been clicking and skipping no matter how much i adjust it the chain cassette wheel and mech is fine so ive no idea what the issue is infact ive. Hecked all the components and wven re cabled it, even when it was working fine i can say it wasnt as smooth as my £70 mtb with tourney infact when i test rode dura ace it didnt change as smooth on the rear as my tourney! As for the front shifting on my sram red well... It will shift to the big chainring once after being adjusted then never again!

Maybe you rear mech hanger is slightly off/bent.
Front mech maybe needs oiling or cable tension/limits are not set correctly.

Im thinking the hanger may be a possibility but its never had a knock or anything to start it doing that, as for the front the limit screws have been adjusted numerous times and its a

Yes - I'm obsessive about the things - same with wheel trueing and brake blocks hitting the rims spot on.

I have 46/34 & 12-28 and even when cross chaining Big/Big & Small/Small there is no rub at all on the FD. Might also help that (AFAIK) my chainstays are fairly short as it is a smallish CX bike.

105 FD/RD and shifts silkily every gear.

My friend has had SRAM Red for years, and even when his bike was brand new it sounded like a bag of spanners when shifting. The hollow, one-piece cassette was a lot of that, but the front changing wasn't great. When I had my SRAM Red it was fine and he was jelly of my gear-changing swag. But now I have a different bike with Force/Red and it's not as good as the Red bike was.

All my SRAM Force & Red kit is never smooth per-se - it clicks & clucks loudly with a notchieness but shifting is crisp & brutally efficient at letting me know EXACTLY when I changed gear. This is why I ride SRAM.

EDIT: I should say these sounds aren't ones of rubbing or failing to catch but rather sounds of something moving & being abruptly stopped. When in gear it's about as quiet as geared bikes get.

Mine was ok, but skipping on 2nd largest sprocket sometimes, so yesterday i fit new cables & had to adjust rear mech a little. It's fine on the 2nd largest sprocket now, but not so good on the 2nd & 3rd smallest  Was getting frustrated, so left it. I'll have another look tomorrow

The older I get, the faster I used to be ...
Yes. Ultegra on my Rose, Veloce on one Ribble, Tiagra on the other, and pretty good on the Peugeot - but that's non indexed down tube shifters, so its up to me. As soon as I hear anything I tweak the cable tension. The Rose and the Ribble were beautifully set up out of the box, and have required virtually no attention.

Yes, Tiagra on my Crux always ran silently and changed perfectly.

I expected the same when I bought my Roubaix with Ultegra, but it always runs fine for the first couple of hours riding then gets a bit noisy and needs constant trimming.

Yep, for the last 8 years my Ultegra triple has been smooth as silk and accurate with every shift. Occasional adjustment needed, but overall a great experience.
The '82 Holdsworth has DT shifters so is great, my Rourke Audax build will also be DA DT shifters with Ultegra running gear so I'm not anticipating anything other than perfection!
