Small front to small rear is a bad combination. the amount of slack you're asking the rear mech to take up is the maximum possible and the chain line will be dreadful, so the problem is not surprising.
The combination of large/ large is also bad, same issues but t'other way round.
In both cases you're much better off changing the front ring you're using to achieve a similar ratio
