My money's on the insert ( rivnut) having come loose- and although still held in the frame it can now rotate. It's happened to one of my bikes. You can re-squeeze the lips of the rivnut together with a Rivnut tool...or use a wheel Q/R (thread is same as a skewer)

It does seem like the threads are worn if the screws keep turning. I would drill out the old thread and re-tap a new thread (so if it was an M5 thread, I'd drill 5.5mm and tap for M6) , however if the screw bosses are made of aluminium you need to be careful not to overtighten the screws, they'll chew up the thread quite quickly.

without seeing the frame or the bolt, it is pure conjecture as to what the problem is.

there will be an insert that is pressed into the frame - either the thread has stripped, easily done in aluminium or the insert is lose; assuming ofcourse that the bolt itself not damaged.

I doubt a helicoil can be used.

The best solution would be a new insert - my money is this is what the lbs will do.
