I've had just the one when my left spd jammed because it was full of dust - on ardnamurchan last year. I hit the deck right in a car park right in front of the local nurse so I got some unexpected TLC, which was nice. Apart from feeling a like right idiot falling on tarmac bloody hurts! Ow!
