Screw the adaptor onto the valve first, then attach the pump to that. At least that's how all adaptors I've used have worked best. They might have given you a Presta-only adaptor by mistake which could be too long to form a seal on some Woods valves but I hope not - they're pretty rare lately.

What pump is it? Can you try attaching its Presta bit?
Thanks for input. The adaptor is correct my pump is Lezyne floor pump and the head is a screw type with no lever locking. Because you screw the head onto the adaptor it tightens it on the valve and jams. I had to remove the adaptor with pliers. Nightmare! Never had a problem with Preta or Schrader! I think these Woods valves suck☹

Getting a pressure reading on a Woods valve can be difficult because the air doesn't blow back in the same way other valves to give the gauge something to measure.
Huh? The pump gauge should show pretty much the tube pressure as long as the pump-valve connection isn't leaking somewhere, as most current Woods valves will only close when the tube pressure is higher than the pump pressure, like a Presta - there is rarely a spring like on Schrader. The captive ball bearing design seems by far the most common now.

Hold the adapter with one hand when screwing the pump on with the other, but even so, if screwing the pump can screw the adapter on tight enough to need pliers to remove it, it's probably pushing metal-metal rather than only pushing on the rubber washer at the bottom of the adapter... so I'm not sure that adapter is correct, but it's odd if it worked at the front. Can you post pictures of the valve head and bottom of the adapter?

I dislike Lezyne pump heads because they've unscrewed some Presta valve cores after being screwed on tight enough to form a seal, but I'm sure I've used them on Woods valves OK, but maybe it was in Presta mode without an adapter.

I've finally cracked it!!! My mistake was using the adaptor when in fact there wad no need?! I simply screwed the Presta pump head directly onto the woods valve and it fitted perfectly forming an airtight seal. I was was able to inflate and psi gauge worked fine.
