Sturmey Archer X-RD3, no middle-gear

Somebody on ebay is selling a cheap Sturmey Archer X-RD3 wheel. The middle gear in the hub can't be selected.
I know that incorrect cone adjustment can cause selection problems for the 1st and 3rd gears but this is the first time that I've heard of a problem with the second gear.
Does anybody know what might be causing this and how easy it would be to repair?

All SA hubs can be stripped won and parts replaced.

I can only think of two things...cable adjustment is out or the pawl springs are u/s or installed incorrectly after a failed strip n rebuild.
