all joking aside when looking at picture i can not help thinking that there must not have been a washer fitted if there was perhaps the arm would not have pulled off, the rx100 is a good crank somewhere like shimano 105, so material should have been good.

That's pretty impressive. It is only ally after all but surely the threads should be weaker?
