Along these lines I think i'm going to be forced into www shopping for new shoes, my current DHB (EU43) have seen better days and also cause me numb feet as they are a bit narrow for me. I've now been to 3 LBS's and none of them have anything wide enough in stock (maybe i have hobbit feet?!).

Have tried Shimano, Specialised, Mavic, Lake & Giro (even the Giro wide fit still pinch a bit)
As regards Mavic.If its of any use,I recently replaced my Avenge with a pair of Ksyrium.I take the same size as yourself and my feet arent narrow particularly,however Ive found that with use over the last three weeks or so,they have relaxed to the point that the ratchet is almost fully home.Now, my Avenge were a superb fit and aside from a difference in the last and the variations in the manufacturing process which may account for some of the variation,I hear (anecdotally,mind you)that Mavic have increased their shoe widths.Have you tried Fizik?I did and found that their 43 was far too wide and also had way too much room in the toe box.Ugly,cheap looking things as well mind you! Perhaps steer clear!

Sidi shoes come up small like Shimano , 43 would be a 44 in Sidi if that's not what you ordered?

I've gone up a size (42 would be normal) and Sidi specify there width as EE to EEE which if correct will be a good fit. So many seem to just quote "wide fit" without actually stating what that is.

The Sidi's have arrived and fit great, no pressure points across the ball of my foot, toes or the bridge. Finally I should be able to ride long distance and still feel my feet!
