Rear derailleur adjustment for dummies

And just to be annoying my shifting has been playing up the last few weeks both directions on the virtuoso so i re-cabled yesterday and it seemed ok for a few miles and now its back to not shifting properly either up or down !
If i can get a gear it is fine with no clicking to indicate its out of adjustment but getting it to select is like rolling a dice and hoping for snake eyes.The cable runs smooth and the cable line through the RD clamp is right .You can see the RD move but sometimes nothing happens or it moves half way and rattles then other times it will engage .
Is the shifter on the way out ?, its a second hand 2300 shifter that i replaced the original with when the LBS told me it was on the way out with similar symptoms .
