HI,

I've not had anything as radical as an osteotomy, but do have pretty lousy knees, with torn menisci in the past.

I have always used Time pedals and cleats, they don't have any adjustment as such (beyond which way round you attach the claets to your shoes) but allow a good degree of float and are double sided and easy to use.

I had them first on an old MTB, but now have them on my road bike, which may not please the purists much, but suit me just fine.

Along with the above advice about correct saddle height etc. personaly it is just about the best I could do I think.
