Just had a thought, do you need a 2 or 3 bolt cleat bolt? I've assumed a 3 but reading a couple of posts I may be wrong and you need a 2?

Had this happen on a TT last year, could hear something "jangling" around but couldn't find it before the start. Set off and got about half a mile up the road and there was a "ting ting ting" of something metallic bouncing down the road, it was the little square plate off shimano cleats. Just carried on and hoped It wasn't anything major (well it was in a TT). I couldn't trace the culprit until some time later when it all made sense. I borrowed an Allen bolt from my mountain bike and a standard circular washer, it's still there now and hasn't been an issue. However it's worth tightening them occasionally to avoid losing them and annoying clicking noises every revolution of the crank which I've also suffered at a different time.

For mtb or 2 bolt..you can use a disc bolt as a temporary fix..if you have disc brakes

i carry a spare cleat bolt...which ive not needed since losing one..

I'll be checking the bolts on my shoes now for sure , as I remember the lad in the shop fitted them , his idea of tight may not be the same as mine , I've not used the cleats much at the moment as I have pedals and shoes that can be used both ways , but well worth knowing , bit hairy coming off and almost being run over ! A dab of silicone would be the go also

A little dab of threadlock is never a bad thing...

From expirience use a decent brand!!
In emergency, nick a bit of SWMBO's nail varnish, works very well, dab on the thread and the chances of loosening are much reduced. All you need to do to remove is a sharp tap with something robust on the end of the socket key and away it comes. And you might get nice colours on the bolts!
