Went to LBS, apparently outer cable length was fine! I haven't got the faintest now!

If it was adjusted so that it changed nicely up from smallest to largest sprockets, then the shift in the opposite direction would be sticky or very hesitant. If I then corrected that, it wouldn't shift back the other way properly again. There was no happy medium. I checked for snagging, made sure the derailleur was lubed up and not sticking - the lot.
Eventually I realised that I hadn't checked the cable inner for corrosion or general gunking up. Undid the cable and slid the outers away, to find lots of dried muck and some very, VERY dry cabling. Cleaned it all out and lubed them and hey presto - shifting issues solved.
