Is there any chance that the clamp and bar was mismatched? I.e. 26mm bar and 1" clamp or vice versa?
Edit: cancel the vice versa.

That happened to me once.
I was using flat bars so it was nothing like as drastic as it would have been if I'd been on the lever hoods with drops. The bars stay in place, and you can steer enough to stop - it's just that they twist freely.
