That's a lucky escape if i've seen one, I've only ever seen that exact type of damage once, from an obvious mistake of over-tightening the bolts out of sequence. - I hope that was the case here more than anything, because if thats down to fatigue then god knows what else is about to go down the same route.

Yikes - I'm glad that you escaped the potentially dire consequences of that!

I had a stem with just 2 bolts holding the front plate on fail when one of the bolts pulled loose (almost certainly due to me overtightening them). Like you, I got away with it. I had just done a high-speed offroad descent so things could have been very serious.

It taught me the lesson that 'b*st*rd-tight' is not a good idea ...

I used a mini torque wrench when fastening it in place about 12 months ago, IIRC to about 6NM, and always tighten up "kitty corner"

What does the inside of the break look like?

I'm wondering whether the clamp was the right diameter for the bars?

Doesn't that look a bit odd? It might be just my eyes but the metal looks thicker at one end of the break than the other.

PS But ... actually, the break isn't perpendicular to the edge of that plate so it probably would NOT be same thickness.

25.4mm bars and clamp. And you're right, the break isn't perpendicular, so there is a slight thickening towards one end.

Its just broken on a different part of the curve. The piece is thinner in the middle than the sides so the thickness varies the further you get from the centre.
