Just as well I wasn't going fast when this cracked on me! Queue much swearing.....
I used a mini torque wrench when fastening it in place about 12 months ago, IIRC to about 6NM, and always tighten up "kitty corner"That's a lucky escape if i've seen one, I've only ever seen that exact type of damage once, from an obvious mistake of over-tightening the bolts out of sequence. - I hope that was the case here more than anything, because if thats down to fatigue then god knows what else is about to go down the same route.
Maybe a couple of cable ties too, just in case.....I think your over reacting, bit of super glue should do it.

I'm wondering whether the clamp was the right diameter for the bars?
Doesn't that look a bit odd? It might be just my eyes but the metal looks thicker at one end of the break than the other.
PS But ... actually, the break isn't perpendicular to the edge of that plate so it probably would NOT be same thickness.
