There is no visible tube for the spacer to go on. It can only go under the stem

Weeellll..... the top cap is only required, in terms of function, to provide the preload prior to tightening the stem bolts. Thereafter it's mechanically redundant isn't it, though obviously rather better for aesthetics and perhaps reassuring in some spurious manner?

Eh? He definitely needs an additional, thin spacer. I'm just suggesting putting it above the stem, not below it.

The shorter stem may make the bars feel lower so an extra spacer may help.
