As far back as possible.
That way I don't hit my heels on the ground on a single track road with a good camber.
I'm on a low recumbent trike ...... ^_^

I just drew an imaginary line between the widest part of each foot and stuck the centre of the cleat right there. To adjust the angle, I used the dangling leg method. Nothing clever, but it seems to work pretty well for me.

It is interesting and thanks for all replies.
I've read the links supplied and yes readjusted so that the cleat is pretty much as far rearward as possible. Interestingly though my shoes don't actually allow for that much fore and aft movement, maybe a centimetre or so. The ball of my foot is over or just in front of the pedal spindle, I think for TT riding it needs to be there rather than the cleat forward in the shoe.

Thank you for putting this post out there. Most of my questions about what I could do have been answered.
 
I think just marginally back of the ball of the foot is right.
I am just moving from SPD to Look Keo type
I saw my existing cleats are giving my left foot a slightly toe in position.
I will avoid this in setting the new ones up. Left knee has been ever so slightly stiff for a while.

It is not just front and back , also side to side and angle as well , all need to be right

Most people like some float -- i.e. the pedal will let you take your natural angle to some degree.
It means the rest of the setup is slightly less critical
