Chain pulling off

This is an annoying video, but this is the principle that I used, I butted the bearings against the C clip. Before this, the old bearing was not flush, I could see this by looking into the frame from the other side. Maybe I am missing a shim, washer from the drive side?

Just a thought, is the point of the H setting also, to stop the chain from falling of when under tension?
