Your age categories are way off. 14-24 goes from people with paper-round money to people who've been at work for years. I'd take the age groups past 44 too. Lots of people buying D-A are going to be older people who can afford the good stuff now the kids have left home etc.
I'd suggest:
14-18
19-24
25-35
36-44
45-54
55-64
65+
Might be worth asking about whether they are employed/student etc. Also, I know you aren't asking for personal info (eg name or address) but it would be good practice to include a Data Protection statement at the end of the survey. Just 'You will not be identified by your responses and we will not pass this information on to any 3rd parties' or similar. Google and you'll find plenty on data protection.
Some of the questions could have more than one response. How will you analyse this?
Why are you asking these questions and how are you planning to use the data? It's easy to rattle up a questionnaire but you need to be sure that you are getting the info that you actually need.
