What is it supposed to be for?
I can't even guess, either from the picture or the term "hanger tool"

It's a tool for not only straightening the hanger but seeing if it's out of line. If you're having trouble indexing the gears this'll tell you if the hanger might be causing the issue. They're easy enough to use if you know how and you can make your own with a bit of help from YT.

And wouldn't a mole or adjustable spanner and a vice be easier

The hanger tool is more about determining if a hanger is bent and by how much and in which direction. It apparently can also be used to determine if brake levers are mounted at the same level.

Several YT channels have featured this recently - even Calvin at Park Tool. You can use the hanger tool to check that your brifters as aligned in both height, angle and distance potentially alleviating discomfort when riding.

So does this one have a means to attach it to the top of the steerer? Mine only has de M10 bolt.

If you need a mech hanger alignment tool, and you also happen to be obsessive about the positioning of your levers it's a win.
