My first ever proper project was writing a stock-control system for National Bus Company in 1980. The designer of the system insisted on a "Are you sure Y/N" response on every input screen. We shared office space with the data-entry team, so were able to watch it in action after the system went live.
For the first few hours, every time the question was asked, the data-entry clerk would carefully review all the entered data on the screen before typing Y and pressing the Xmit button.
After that the enter sequence was Xmit-Y-Xmit without once looking at the screen or interrupting their chat with their neighbour
