Thanks, @mcshroom, you've summed up where I am pretty well. I'm sure that a 42 large ring must be well outside the published range for a 2203, but with careful positioning it does seem to cope.

I can fit either a top- or bottom-swing mech OK, though I would've needed a couple of washers and a longer bolt to keep the mudguard clear of the top-swinger. But I'm not going down that route for now.

I'll keep a lookout for some bar-end levers. I know a few places that have good quantities of reconditioned parts. Thinking about it, I still have the V-brakes that came with the frame, so that option's starting to look more tempting.

If you do, I recommend the Tektro RL520 levers. They work well, and also have a quick release in the lever which means you can release the v-brakes.
 
Also if you do go the MTB front derailleur route, you may need a 12 tooth difference between the middle and the big chainring.
