Please don't tell me that this is a simple job! I poked out the old cable and watched where it came from, the lever was all the way over to the smallest chainring. Threading the new cable should be a simple matter of threading it in the reverse of the old one, right? The problem is that there is no hole at the end, I can trace the route of the cable backward through the outer casing to the lever over the changing mechanism but there is no exit, is there something I am missing? I am very frustrated with this.

This may help.
If it's the left hand lever then you need to push the smallest lever to the lowest crank setting before threading.
 
The hole should become visible when the lever is moved to one of its extreme gear positions...
Although when I last did it I kept swinging the lever slightly when 'opening it' and obscured the hole I was looking for.
I wonder if that is what is happening.
