I suffered for years with a very similar set of circumstances and the pain getting gradually worse every year. My doctor at the time blamed a very heavy job I had early in my career which admittedly did give me occasional hand injuries and basically said I should put up with it.
2 winters ago I went to the doctor for some pain relief and was met with a different response. My old doctor had retired and my new doctor was astounded when I explained my symptoms and the previous response, as was the consultant he referred me to.
Turns out I have psoriatic arthritis. They tried me on Methotrexate which didn't really work for me and I am now on a drug trial instead. The trial drug has completely eradicated my symptoms so fingers crossed it is licenced prior to the end of my trial.
Personal experience obviously but I suppose the message is don't give up at first diagnosis if your in pain, there appears to be little that cannot be helped by the correct medical professional.
I would still like to see my old doctor that ignored me for all those years so that I can show him what a good job his replacement has done by squeezing him gently by the throat with my now fully functional hands
