Tektro Rear disc brake failing

Being new to the world of disc brakes on bikes I need some advice. My rear Tektro Lyra disc is causing my problems, occasionally under braking I hear a click and loose all braking (as if the cable had snapped - which it hasn't). If I release and reapply the brake it works. I've tried adjusting the inside 5mm allen bolt and the cable adjuster and it seems better, but still happens.

The pads have done about 1500 miles, could it be that they need replacing?
