Followed the instructions that suggested I try different positions then fully tighten the cleat when happy. Clipped the shoe into a test position, but the cleat isn't tight enough and moves with the shoe when I try to unclip.

Can't get at either the cleat screws or the tension screw on the SPD. How can I unattach the shoe?!

(pedals are M520s)

This is why I hate DIY...

put them in the last tested position then tighten fully...carry the allen key and adjust your cleats when you think one needs moving forward/back/right/left. It takes a few trips, -my left was spot on straight away but my right needed 5 trips out.

Ta, got it in the end, a combination of the above and took the pedal off, put it in a vice and twisted the shoe. Job done, doesn't appear to have caused any damage, lesson learned.

They should put clearer instructions on those things for first timers :/

Out of interest, is it normal to have the cleats in similar position on each shoe, or can the comfortable position end up being quite different?

The latter. Obviously, the cleats will only be in the same position (correctly) if your legs are symmetrical and you ride in a bio mechanically symmetrical way. Many people don't. Mine are in slightly different positions - I broke my ankle a few years ago, and I heel in more on one side - you can tell from the scuffs on the cranks!
