spacers tightness in the handlebar

Hello folks
when I open the handlebar and put the spacers on the bottom to make it higher. Ifeel that after everything is back and screws are tight that the spacers are still loose. anything I did wrong?

Threadless stem. Leave the Allen bolts loose on the stem and then tighten the top cap, but not too much. Now tighten the stem bolts while aligning it with the front wheel.

Thank you @Cycleops I tighten the cap very much. I went back and loose it a bit.
