Last year I suffered with a sore outside of the foot after a lengthy ride. This would persist off the bike, making walking quite uncomfortable. The pain is along the outside of the foot, under the ankle, with ankle itself also being sore. And slightly swollen.

This year it's started to happen again, only this time on the other foot. I ride road bikes and always use SPDs. The pain is there no matter which bike I ride, or which shoes I'm wearing.

Does this sound like a bike set up issue? In every other respect the bikes are comfortable so I'm not sure that is the problem.

Anyone got any ideas?

SPD's aren't best for long rides - assuming MTB SPD's.

TBH I use MTB SPD's all week every day, but on my road bikes use Look road systems - but don't have any issues.

Are your shoes supportive enough ?

Have a look at Specialized Body Geometry insoles, they come with thin wedges that you use to improve the angle of your foot on the pedals. I had a similar problem to you; my foot felt as if it wanted to slip off the outside of the pedal and was sore on the outside; a bike fit specialist lent me some wedges and I soon found they helped a lot so I bought the blue (medium) insoles. They are very comfortable and combined with a proper road shoe and Look Keo pedals they feel like a good stable platform for out of the saddle efforts.
