No issues heard of. If you took it into a bike shop i'm pretty certain they would say it was tightened incorrectly.
Did you fit it? or have you tightened the bolts yourself?
 
Yep fitted myself as per Deda written instructions using torque wrench. Equal spacing, stepped torqing, etc... Been on bike for 2000miles....
Send it back with a polite covering letter and keep your fingers crossed is probably the only course of action IMO.

I would hope that Deda are understanding and compliant, but I fear DIY fitting, (regardless of procedure), will be an easy get out for them.

For instance, (and I'm just playing devils advocate here), if they ask for a dated calibration certificate for your torque wrench, could you legitimately supply one?

Its a alloy stem which is not manufactured using any unique process so I'm not sure what part of that is cutting technology.

When you start looking into Deda stems around other forums, you can quite easily find reports of face plate failings going back over a period of time, I'm not suggesting anything from this im just observing its interesting the number of face plate issues you can find, and its not exclusive to one stem model, one such example is shown below.

Here's a pic of the offending item for your perusal.
60 seconds prior to it letting go i was going downhill, in traffic, at 35mph.
Will be paying closer attention to component condition in future.
