I have (after a puncture) a small tear in my rear tire - its about 3mm long, and when inflated it opens up to about 1mm wide, though the gap is larger on the outside of the tire than the back/inside, for example I cannot see though it and I cannot see the innertube. On the inside of the tire Iv put a few layers of insulation tape just incase.

Is something like this a concern, should I look to replace it?

Should be Ok but check it regularly in case it gets worse, as long as the innertube is not visible thru the hole then I would continue to use it, especially on the rear.
Put a pic up if you can.

I think I'd look upon it as a temporary bodge while found a reasonably priced replacement. There are enough deals around not to have to ride too long on an iffy tyre in my opinion.

i've had blow-out at high speed caused by the inner tube splitting through a split tyre and it's not nice. Definite new tyre time I'm afraid

If the tyre's only used for local riding, it can still have many miles in it. But insulation tape isn't going to make any difference, you need something stiff, thin and flexible. If the carcass is damaged in any way, the tube will eventually find its way out and puncture, unless you've something stronger than insulation tape.

My rear has a 6mm a 1mm gash in it and its gone through the carcass, albeit only a little. With reinforcing, its lasted maybe 1000, 2000 miles...and i do long runs on it. Should listen to my own advice and only use it locally, but...

A patch of leather glued to the tyre inner is how we used to do it. The tongue of a boot is ideal!

I never tried this, but why not use a puncture repair patch applied to the inside of the tire to add some strength around the hole. I think it would help until you're in a position to replace the tyre.

I've had tyres 'blow out' before so I'd look to be replacing it if it was me. You could of course put a patch on the inside of the tyre to secure the tear but that would only be a temporary measure.

The choice of course is yours
