securing cable housing to brake lever

The cable housing for one of my brakes is not being gripped properly by the adjuster barrel (picture below) and it comes loose. How do I fix this?

tbh it appears that there is something amiss wit the rest of the brake system. Normally tension on the outer casing between the barrel adjuster and mounting points keeps everything in place. Check to make sure nothing has come adrift. Alternatively it may be that the outer has been cut too short.

As Andy R writes, the other end of the cable may have come loose from the cable stop... This would be the first thing to check.
