Problems in lowest gear

I was out today and on a difficult climb I went into my lowest gear(shimano tiagra) and it was making very strange noises and the chain seemed to be slipping.

Any ideas what the problem could be?

I just had a closer look then and the chain doesn't seem to be to sit on the top ring in the cassette

Need to be more specific. Does it only do this on one chainring, or one sprocket? Is the chain worn? Do you rarely use the sprocket or chainring that it slips on? It could be that the chain has worn and no longer properly fits sprockets/chainrings that you rarely use.

Im not sure about the chainring, I bought the bike around the middle of june and have ridden about 2000km. I rarely use my lowest gear as I don't live in a hilly area.

Does it look like this?
A worn chain on a new sprocket. Due to the pitch mismatch, the load is only carried by the teeth/rollers on our right, the chain hangs slack on our left. The new sprocket will wear rapidly to match the pitch of the worn-out chain.
