I have the well known symptom of a 'clicking' noise coming from the bottom bracket. I have had the same noise before and found replacing the BB cured the issue. The BB is a Tiagra 4500 and I have read that the bearings can get an ingress of water over time. I have done about 4500 miles since the last BB change.

Enough of the waffle, I will cut to the chase ...

When I install this particular BB, which I beleive is sealed, do I need to grease the crank shaft and the inside of BB or do I just clean it all and put it back together 'dry'?
 
4500 miles? Your BB is still in it's nappies!

Just try removing it, clean the threads in the shell and on the BB as well as you can, add a bit of anti-seize, and pop it back in.

Don't forget to take you time when you refit it - cross threaded BBs can be a very depressing experience.

I've just fitted one of these and spoke to the LBS about it, he said be prepared to take it out, clean it, re-grease and re-fit every month or so or as the clicking occurs. I used babywipes to clean the Deore BB on my MTB recently and put grease back on all of the bits, not too much though or it's like pedalling through treacle! I add grease as an extra layer of protection, but I believe the best remedy for these is to clean them often.
