it sounds like your existing headset is goosed ,if it was me (and the existing forks are ok)id just fit a new headset and jobs a goodun ,it all depends on the length of steerer u need as no doubt they (new)ones will need cutting and rethreaded
 
It should be straight forward. Just check you have assembled everything in the right order.

1 Crown race onto forks at base of steerer - should be a snug fit.

2 caged bearings

3 Lower race (which fits in head tube)

4 Upper race (which fits in top of the head tube).

5 caged bearings.

6 Adjustable race

7 Locknut.

Then tighten up until there is no play (make sure that there is no play in the brakes!).

May I add, if that doesn't work, get the LBS to check it out. Just a suggestion.
