Is this cable guide riveted on?

Large pop/snap head rivet. Drilling out is the only way to remove.

Respray, or if it's loose. You can't tighten it.

No need to fix it back, the cable tension will keep it in place but if you’re worried a large self tapper would work.

1/8th drill bit, and at a minimum just the head needs removing.

Quite often the hole is threaded.
A new one only costs coppers and will come with a fixing bolt.
