I keep coming un-clipped

Earlier this year I bought a pair of Specialized Tahoes which I fitted with new SD cleats.

For some reason these keep coming unclipped from my pedals - it feels as if they never quite lock in fully. In contrast my old Shimano shoes, which I still sometimes wear, clip in really firmly.

I've tried tightening up the tension bolts on the pedals but that doesn't seem to make any difference.
 
Are they Shimano cleats or lookalikes, assuming you have Shimano pedals. Tightening the tension never makes a difference, that only effects the release action.

I have had lookalikes.. and they were shiyte... can't remember where I got them from but they just didn't hold as securely as Shimano cleats. Live and learns and all that

Dirt, worn cleats, cleat too close to the edge of the recess?
Not dirty or worn.

What do you mean by "too close to edge of the recess"? The cleats do seem to be more sunk into the recess than they are on my old Shimano shoes - could that be causing the problem?

I had to put a thin spacer between shoe and cleat on one combination that I have. Long way from home and I cannot remember the shoes but it is with SPD.

You often need shims under the cleat with crank brothers.. not needed them with shimano tho,I have 3 pairs of shimano spd shoes and all work well with shimano cleat,idont bother with coppies on cleats what ever version.
if this is mtb cleat have you got the sh56 version as they are for a more novice or nervy spd users like me..after a few wtf moments...I liked them for learning..
if yes get the sh51 as they are better for keeping clipped.
I find my spdsl cleats very firm in contrast but then I dont sudenly decend on rooty mudy trails when im using them....

I was thinking too far left or right so that the edge of the recess interferes.

Ah - they are SH56.

I wasn't aware there was more than one type of Shimano SD cleat - I must have randomly bought SH51s when I changed my cleats in the past.
