You need insoles which make the space between the shoe and arch close. I got specialized ones - if you go into local bike shop they have a gadget where you put your feet on and it measures where your arch is, and where it should be.
I got some 1 degree wedges that fit under the cleat. to be correct I have 2 degrees under my right and 1 degree under my left foot. it means that your foot will place flat inside you shoe and not put all the pressure on the outside of the foot. you can get them from wiggle and chain reaction cycles for about £12. before I used them my foot pain would leave me unable to walk.
