Help with slipping drive train on trike

I've recently acquired a delta style trike. It was almost certainly made in china. I've checked the headset and bottom bracket - very little wear, I don't think the trike has seen much use. I've upgraded the front brake and fitted an electric bike kit that drives the front wheel. When I ride the trike I'm aware that the crank seems to slip about a quarter turn, this only reoccurs if I have used the rear brake. I've checked the axel and drive wheel and the problem is clearly the cheap and nasty way the axel and drive wheel mate. Pics attached to show axel and what is supposed to be the 'flat' on the wheel hub! Being unfamiliar with trikes is this the standard way a drive wheel fixes to the axel? I've tried to improve the drive by fixing a washer to the wheel hub and tab - see pics, but it couldn't take the load. Any suggestions as to how I can improve the situation would be welcome, thank you.
