I used to put a flat bit of wood over mine then tap it carefully home with a hemmer. I think Sheldon Brown approves the same method. Not by the book, but with care it works.

I've got a home-brew version but paid for a cheap £20 job. It's way easier, faster and does by bb bearings too.

I welded one up. It works for me on the odd occasion I need to do one. The handle is threaded.

Edit. The tube sections are actually bits of an old steel handlebar from a discarded and wrecked BSO.
