Headset conversion (please go easy on me)

I have a one piece riser and handle bar stem on my bike which is damaged,
I would like to convert to riser and clamp on handlebar stem the bike is quite old and the headset is 1 inch and I take it it is likley the forks are threaded. Do in need one of these for the conversion http://www.ultimatepursuits.co.uk/products/details.aspx?ref=14132&pRef=33063&vertical=1&v

Will i need to to do a headset conversion as well
