When gear change get's sloppy or stiff. If you feel it's not changing well, just change the cables (and outers) - then you'll really notice the change.

As for when/how often, this depends upon your riding, what weather conditions it's ridden in, and of course how you maintain/clean the bike.

In my OH's case, when they break while she's riding the bike, and she has to ride all the way home stuck in the highest gear on the cassette. It probably won't teach her, though ....

That's been my experience the last few times - both bikes.

I don't know what's changed, but I always used to fell them going, normally not changing to higher gears (rear) or lower ones (front) quickly and sharply, but the last few have just failed with little or no warning.

NO! Not when the inner snaps.

The outer cable that curves around to the rear derailleur will wear pretty fast and get clogged with road dirt and rust and and shifting becomes stiffer and less accurate as the weeks go by. A new outer cable every six months will transform your gears and you'll be amazed at how much you have come to accept bad performance as the norm.
