Foot pain

So I get cramplike pain in my right foot whilst cycling on my roadbike. I have a long term hip injury (rugby is a fun but dangerous sport!) which has resulted in me shortening and tightening all the muscles used for external rotation. Basically means that when standing upright I cannot turn my foot out like you would to walk like a penguin. My physio has suggested that the pain main be due to my cages putting to much pressure on the side of my foot forcing the muscle to work extra hard thus the pain is compensation. So the question is do I

a) take off the cages
b) move the position of the cages (and if so how?)
c) remove one cage and leave the other one (will this cause balance/power issues?)

Although I would like to go clipless the heel twist to remove/engage them is impractical due to injury and woud just increase my risk of further injury or fall.

Can you get clipless that you slide forward into and backwards out of?
