I’ve not had any loosen, and have usually added copper grease to aid any future disassembly.

Clean n dry. A happy medium between greased and threadlocked.

Yes.
Once fitted never taken off. Same argument as for chainring bolts, bottle cage screws, cleat bolts and mudguard stay fastenings (at dropouts, not up by guard, unless you're a 'mudguards on, mudguards off, and repeat kindarida).
