It's seems odd that if an accident can be described as "tragically common", there was no element of said accident that could give the coroner reason to think it "illustrated a systemic problem or that it might call for some specific response"(my bolds).

That's shocking O. I do wonder when the judicial system and policy makers will wake up to the importance at looking at this.
