I've created a Cyclist Down forum and moved all related threads into it.
The forum is specifically for reports / discussions of incidents where cyclists have died or been injured.
For those who wish to use this forum, please be sensitive and respectful when posting and try to avoid wild speculation (you sometimes have to be patient and wait for information to come out).
For those who do not wish to read this forum, you can exclude it as follows: click your username (top right), select What's New from the options list, scroll to the bottom and tick 'Cyclist Down', then click the Save button.
